RE-ISSUE OF PASSPORT
Please apply for RE-ISSUE of passport in the following situations:-

(1) When validity of the passport has expired (normally a passport is valid for ten years. For a minor up to 15 years, passport is valid for 5 years).
(2) When the validity of the passport is due to expire within 12 months.
(3) When the pages in the passport have got exhausted.
(4) When there is a change in appearance (with /without turban or with/ without spectacles etc.)
(5) When deletion of Emigration Check Required stamp is needed.
(6) Loss or damage of current passport.

Checklist for submission of application for re-issue of passport

Application form has to be filled online in the below mentioned portal https://portal3.passportindia.gov.in/Online/index.html

1. Printed online application form.  [After filling details online, the application needs to be printed and should be submitted with fee and supporting documents at the Embassy counter. All columns of the form to be completed in block letters. Signature to be signed (or thumb impression to be affixed) within the box in the first page without touching the lines].

2. Four passport size photographs (size 5.0 cm x 5.0 cm with light background showing front view of face). One of the photos is to be pasted on the first page of the application form. The remaining photographs should be submitted intact (please DO NOT staple the photos or sign over them). Photograph specifications can be seen under the section ‘download forms’

3. Photocopy of the current passport including photocopy of endorsements, if any, done by Indian Passport Issuing Authorities.Current passport is to be shown, in original, at the counter at the time of submission of application and taken back. The Embassy will not retain the original passport of the applicant.

4. Photocopy of the Residence Permit in Belgium / Luxembourg. Original Resident   Permit to be shown at the counter.

5. Prescribed Fee(As per fee schedule given at the end of the page.)

 

Application for Re-issue of passport of Minor Child

Application has to be filled online URL https://portal3.passportindia.gov.in

Printed online application form.  [After filling details online, the application needs to be printed and should be submitted with fee and supporting documents at the Embassy counter. All columns of the form to be completed in block letters. Signature to be signed (or thumb impression to be affixed) within the box in the first page without touching the lines].

  • Four passport size photographs (size 5.0 cm x 5.0 cm with light background showing front view of face). One of the photos is to be pasted on the first page of the application form. Thumb impression/signature of the child has to be affixed in the application form below the photograph. The remaining photographs should be submitted intact (please DO NOT staple the photos or sign over them). Photo specifications can be found under the section ‘download forms’
  • Photocopy of the current passport including photocopy of endorsements, if any, done by Indian Passport Issuing Authorities. Original passport has to be shown at the counter.
  • Photocopy of the Residence Permit in Belgium / Luxembourg. Original Resident Permit to be shown at the counter.
  • Copies of passports of both parents (Originals to be shown at the counter). It may be ensured that, at least one of the parents’ passport contains spouse name.
  • Copies of resident permits of both parents (Originals to be shown at the counter)
  • Joint declaration by the parents (when both are residing in Belgium / Luxembourg) (Kindly download form from website- application forms)
  • Prescribed Fee (As per fee schedule given below)
  • Application for Citizenship and Fresh passport for a New born baby

    Provided at least one of the parents is an Indian national, a baby born in Belgium / Luxembourg can be registered as an Indian national within one year of the birth of the child. With effect from 1.7.2012, it is mandatory to file an online application for grant of Indian citizenship by descent under Section 4(1) of the Citizenship Act, 1955.The online form is available at http://indiancitizenshiponline.nic.in/ under the Foreigners Division page of the website of the Ministry of Home Affairs, New Delhi. If the application is made within one year of his / her birth in the prescribed form at the Embassy of India, Brussels, then the Embassy shall provide the Indian citizenship to the new born baby. In case the application is made after one year at the Embassy of India, Brussels, then the application would be sent to the competent authority for their approval.


    Checklist for submission of application for a fresh passport for a newborn baby born in Belgium / Luxembourg.

    1. Hard copy of the online application (Form-I) for grant of Indian citizenship by descent under Section 4(1) of the Indian Citizenship Act, 1955. The online form is   available at http://indiancitizenshiponline.nic.in/All columns of the form to be completed in block letters. Photograph of the baby and parent’s signature has to be scanned and uploaded in the online portal. Also applicants has to upload all supporting documents in the portal.

    2. Printed online application form for passport. Online application form for new passport has to be filled up online; https://portal3.passportindia.gov.in/Online/index.html Thumb impression of the baby has to be affixed in the first page of the application form and Parent’s signature has to be given in the application form in the given column.

    3. Four passport size photographs (size 5.0 cm x 5.0 cm with a light background showing front view of face with eyes open).One photo each is to be pasted on the        first page of the passport application form and on the hard copy of the online form.             The remaining photographs should be submitted intact.

    4. Joint declaration by the parents (when both are residing in Belgium /Luxembourg) –Form can be downloaded from the section ‘download forms’.

    5. Photocopy of Birth Certificate of the baby (original to be shown at the counter).  Please submit an international extract for birth certificate, or one that is in English.For Brussels, following link may be useful:https://irisbox.irisnet.be/

    6. Photocopies of passports of the parents (original passports to be shown at the counter). It may be ensured that, at least one of the parents’ passport contains spouse name.

    Loss / Damage of Passport

    Checklist for a new passport in lieu of a lost / damaged passport

    Application form has to be filled online in the portal

    2. Four passport size photographs (size 5.0 cm x 5.0 cm with alight background showing front view od face). Three photographs to be pasted on the passport application form at the relevant pages. The remaining photographs should be submitted intact (please DO NOT staple the photos or sign over them).

    3.  Police Report(FIR) clearly mentioning the Passport Number

    4.  A statement indicating circumstances of loss / damage of passport.

    5.  Photocopy of the lost / damaged passport, including photocopy of endorsements, if      any, done by Indian Passport Issuing Authorities.

    6. Photocopy of the valid visa / Residential Proof in Belgium / Luxembourg.

    7.  Any photo identity proof (if sought by the Embassy).

    8.  Prescribed Fee (As per fee schedule given below)

    Change of Name

    In addition to the documents mentioned above in the check list for Re-issue of Passport, following additional documents will be required for change of name in the following cases.

    When a person has no Surname and wishes to split his "Given Name" as "Given Name" and "Surname"

    Sworn Affidavit stating the intention to change the Name.

    Two public documents issued from India with Name (Given name and Sur Name written in row or separately)

    (Birth certificate, PAN Card, Aadhar Card, School Leaving certificate etc.) Original public documents has to be produced at the counter for verification.

    If two public documents are not available, kindly follow the procedure for Change of Name by submitting Two original newspaper advertisements announcing change of name (circulation of the newspapers in the area of permanent address in India and present address in Belgium/Luxembourg).Kindly wait for a period of one month after
    Insertionof advertisements.

    Addition of "Surname" where a person previously only had "Given Name".

    Change in "Given Name" / "Surname".

    1.Sworn Affidavit stating the intention to change the Name.

    2.Two newspaper advertisements about change of name (circulation of the newspapers in the area of permanent address in India and present address in Belgium /Luxembourg). Kindly wait for a period of one month after insertion of advertisements.

    C. For Ladies, change of name from maiden name to married name.

    1. Sworn Affidavit stating the intention to change name

    2.Copy of Marriage certificate(Original has to be shown at the counter) and Spouse Passport copy

    D. For widows / divorcees, change of name on re-marriage.

    Sworn Affidavit stating the intention to change name.

    Original Divorce deed / Death certificate/Marriage Certificate and spouse passport copy, as the case may be (Original has to be shown at the counter).

    E. Married women wishing to revert to maiden name when not divorced or widowed.

    1. Sworn Affidavit stating the intention to change the Name.

    2. Two newspaper advertisements about change of name (circulation of the newspapers in the area of permanent address in India and present address in Belgium /Luxembourg). Kindly wait for a period of one month after insertion of advertisements.

    Civil documents, e.g., Birth Certificate, Marriage Certificate, Death Certificate, etc. should be apostilled by Ministry of External Affairs, New Delhi. Also, all the documents in other than English language are to be translated into English by an authorized translator.)

    Police Clearance Certificate (PCC)

      (Foreigners who have previously resided in India may also on occasion apply for PCCfor obtaining visas for certain other countries which require such documentation. Foreigners has to fill up manual application form with all supporting documents like PP Forms etc.)

    As per latest instructions, For issue of PCC, fresh police verification has to be carried out in India. Kindly apply well in advance, as the whole process may take 4-6 weeks.
    If an applicant while applying for PCC is providing the details which are different to details available in the passport already held by him/her, first of all, the applicant has to apply for re-issue of passport with new particulars.
    PCChas to be applied after re-issuing passport with new particulars.

    Emergency Certificate
    Emergency certificate is issued to Indian nationals in certain circumstances, to enable them to return to India. EC is also issued to those citizens of India to whom a new passport cannot be issued because of pending verification from India or who are under orders of deportation. EC can be issued only when the individual’s Indian nationality is confirmed.

    If original Indian passport is not available, Police complaint/FIR mentioning the Passport Number and a self-affidavit explaining the circumstances of loss of passport.

    Renunciation of Indian Citizenship and Surrender Certificate for those who have acquired foreign Nationality on or after 01/06/2010.
    (The process of Renunciation of Indian citizenship has changed with effect from 21/08/2021)
    Application for Renunciation has to be applied online in the Ministry of Home Affairs (Foreigners Division) website.

    The applicant has to click on “Declaration of Renunciation of Citizenship under Section 8 of the Act Made by a Citizen of India who is also a Citizen or National of another country
    The renouncer would fill all his/her details and upload relevant documents in above mentioned website, take a print-out and visit the Embassy of India Brussels for signing the declaration before the Consular Officer. Appointments may kindly be scheduled for visiting the Embassy for submission of online application form and signing the declaration.

    Once the application form is submitted online by the applicant, then no furthermodifications are allowed. Hence applicants are requested to carefully verify thedetails before submitting the online

    Surrender Certificate for former Indian nationals, who acquired foreignnationality before 01.06.2010) :

    3. Nationality Certificate indicating exact date of acquisition of foreign nationality from the concerned commune.

    5. Current Belgian/Luxembourg passport and photocopy
    6. Original Indian passport and photocopy of the passport, including endorsements, if any, done by Indian Passport Issuing Authorities.
    7. Photocopy of the Residence Permit in Belgium / Luxembourg. Original Resident Permit to be shown at the counter.
